The claim should have been submitted my new insurer Empire BCBS instead of the old Allied Benefits.

I spoke to Kelly.
She said that when I gave them my new insurance info, Empire had incorrectly been added as secondary insurance.
She will resubmit the claim to Empire, so I (hopefully) won't have to do anything else.
